Rockford Thunder Appearance Request Form and Information
The Rockford Thunder is currently scheduling player appearances for the upcoming months and we want to offer you the opportunity to schedule one of our players to make an appearance at your event.
Players currently living in the market and available for appearances are Kristen Zaleski and Jamie Davison. Other players can also be brought in with the appropriate amount of notification. Front office personnel are also available if requested.
The following information is necessary to include when requesting a Thunder representative to make an appearance for you:
Organization name
Contact name
Address of the event
Directions
Contact phone numbers
Email address
Date and time of the event
Explanation of what the player is to do at your event.

PLEASE NOTE: Completion of this form indicates a request, it does not guarantee an appearance. Appearance requests must be received a minimum of ten (10) days prior to the event to be considered. Incomplete forms will not be considered.
